New Patient Registration

When you register with this practice you will be asked to complete a short, confidential questionnaire. 

Registration Process

Attend reception and complete a GMS1 registration form and a patient health questionnaire. Your registration form will then be passed to our administration team who will create a medical record for you on our clinical system.  This will send a link to the health authority who will request your paper medical notes from your previous surgery (patient notes can take a few weeks to arrive).

Please note our surgery has the facility to request your notes via GP2GP, this means that your electronic notes will be called and put into our system within minutes of the administration team registering you.

If you are taking any repeat medication, please supply us with your latest repeat medication slip from your previous doctor. The Doctor will put your medication on repeat prescription.  The Doctor will arrange an appointment for you if they think a medication review is required.  Please advise us if you would like to collect your medication from the surgeries elected pharmacy or a pharmacy of your own choice.

When your paper notes are received by the surgery your notes will be summarized and your electronic record updated with the necessary details from your paper notes.

It will be identified from your paper records whether you are required to attend any chronic disease management clinics and if so, the relevant nurse will be notified and you will be invited to attend a clinic when a check-up is due.

Please note that you we will not be able to issue medications to you as a newly registered patient until after at least one week. This timeframe is necessary to ensure that any medications you are being prescribed are recorded correctly onto your medical record and any queries can be administered by the surgery staff. In the interim period you will be required to contact your previous surgery.
