As a patient you have the right to request access to your medical record.

Online access to your GP records is free of charge - please see our 'Patient Online Access' page.

You can also either request an appointment to view the electronic record held by the surgery or request paper copies of your medical record.

You will only be charged if the subject access request is 'manifestly unfounded or excessive'.  In this case you may be charged a 'reasonable fee' based on administrative costs of providing the information.  If this is the case, you will be given an explanation and given the information to complain to the Information Commissioners Office (ICO).

All requests are received by our Care Navigator Team Leader and processed accordingly.

If you would like to gain access to your medical record you should complete the following form (or ask for one at reception) and forward this for the attention of Care Navigator Team Leader, Riverside Surgery who will then contact you by telephone to discuss the process with you.
